This web site is designed as an area where Certified Athletic Trainers at the High School level can share documents specifically for the High School setting. Below is a description of each area of this web site.

Parent Forms: Documents in this category are those that you send home to parents. For example, permission to treat, information about the athletic training program, etc... (LAST UPDATED 2/8/06)

Policies: Documents in this category are policies for the athletic trainer, parents or coaches. For example, policies and procedures manuals, standing orders, AED policies, coaches emergency procedures, etc… (LAST UPDATED 9/30/03)

Reports: Documents in this category are any type of report forms that you use in your setting. For example, injury report forms, evaluation forms of you or your staff, etc... (LAST UPDATED 2/7/07)

Physician Forms: Documents in this category are any type of forms that you have made for communication with your physician. For example, you might have a form that your physician must fill out each time your athlete visits the office. etc.. (LAST UPDATED 2/15/05)

Physical Forms: Documents in this category are any type of physical forms your school district uses for student-athletes. (LAST UPDATED 2/8/06)

Student Forms: Documents in this category are any type of forms you use for your student assistant program. For example, syllabus from class, evaluation of your students, etc... (LAST UPDATED 9/30/03)
